A surveyor is not looking for your binder. They follow a patient.
The tracer
An individual tracer picks a patient and walks the path: admission, assessment, medication orders, transfers, discharge. At every step the surveyor asks the person in front of them how do you do this here and then compares the answer with the policy and with the record.
The three questions
- Show me. The document, the form, the training record.
- Tell me. Can a nurse explain the two patient identifiers without reading them?
- Prove it happened. The completed form, the KPI, the CAPA that closed the last finding.
Why mock surveys fail
Most mock surveys are too polite. They are announced, they visit the best unit and they score generously. A useful mock survey uses the scheme's own scoring rules, samples units at random and produces a report that ranks findings by risk.
Turning findings into readiness
Every finding gets an owner, a due date and an effectiveness check. Track the closure rate weekly; a facility that closes findings within 30 days rarely fails the real survey.
